If you are requesting an annual donation record, please upload copies of all BTWHSPVA PTSA-related donation receipts from the previous calendar year that you would like included.
Please note | All donation record requests must be submitted by January 31 each year for the prior year’s documentation. Requests submitted after the deadline should include any extenuating circumstances for consideration.
